AVFilterLink.frame_count is supposed to count the number of frames
that were passed on the link, but with min_samples, that number is
not always the same for the source and destination filters.
With the addition of a FIFO on the link, the difference will become
more significant.
Split the variable in two: frame_count_in counts the number of
frames that entered the link, frame_count_out counts the number
of frames that were sent to the destination filter.
Libav, for some reason, merged this as a public API function. This will
aid in future merges.
A define is left for backwards compat, just in case some person
used it, since it is in a public header.

With the introduction of AVFilterContext->is_disabled, we can simplify
the custom passthrough mode in filters.
This commit is technically a small compat break, but the timeline was
introduced very recently.
Doxy by Stefano Sabatini.
This should fix several issues with odd dimensions inputs.
lut, vflip, pad and crop video filters also need to be checked for such
issues. It's possible sws is also affected.
This is mostly automated global search and replace
The deprecated aconvert filter is disabled, if it still has users
it should be updated

This also drops support for "flat syntax" and "reinit" command.
"reinit" command is not very robust and complicates the logic more than
necessary, since requires to reset all the options in the command.
*This is a syntax break*.
Parse expression only when a new value is explicitly specified.
In particular, avoid double free in case an old expression value is
cached, it is set in the context, it is freed as the old value, and
finally the pointer stored in the context is freed again when the filter
is released.
This allows dynamic reconfiguration of the filter.
The callback uses some code that was in the init function. Hence this code
has been moved in its own function.
