dprintf clashes with POSIX.1-2008

This also lists the objects from those two libraries as internal (by adding
the ff_ prefix) so that they can then be hidden via linker scripts.

Set service_provider and service_name in mpegts demuxer, previously
name and provider_name were set but since the muxer uses service_provider
and service_name use them.
