Currently FFM files generated with one versions of ffmpeg generally
cannot be read by another.
By spliting data into chunks, more fields can saftely be appended to
chunks as well as new chunks added.

Regression since 5f847bf61d.

After this commit, timestamps pushed by FFmpeg won't be relative
anymore, but absolute (based on the date/time at the beginning of the
push). This will allow seeking to work properly. Before this patch, the
seek was done, but ffm timestamps were way smallers than the absolute
requested timestamp (based on a date), so the seek was done, but to the
end of the stream (which was similar to no effect at all).

This is consistent with stdio and is what we want to do in all cases.

Fixes a bug in the voc muxer which didn't flush in write_trailer()
previously. This is the cause of the change in the test results.

The condition is trivially true, but keeping the assert() is
sensible to avoid FFM_HEADER_SIZE ever getting out of sync with
the actual code.

Deprecate corresponding AVCodecContext fields.

Also remove some pointless NULL/0 assigments.

C++ code must be left as it is because named struct
initializers are not supported by C++ standard.

The existing functions defined in intfloat_readwrite.[ch] are
both slow and incorrect (infinities are not handled).

This introduces a new header with fast, inline conversion
functions using direct union punning assuming an IEEE-754
system, an assumption already made throughout the code.

The one use of Intel/Motorola extended 80-bit format is
replaced by simpler code sufficient under the present
constraints (positive normal values).

The old functions are marked deprecated and retained for
compatibility.

Deprecate AVCodecContext.directpred

Deprecate AVCodecContext.partitions.

It's more readable and less prone to breakage.

AVStream is no place for it and it's unused outside of ffmpeg anyway.
