Despite its name, this function is not part of the public API, as
formats.h, the header containing its declaration, is a private header.
The formats API was once public API, but that changed long ago
(b74a1da49db5ebed51aceae6cacc2329288a92c1, the commit scheduling it to
become private, is from 2012). That avfilter_make_format64_list() was
forgotten is probably a result of the confusion resulting from the
libav-ffmpeg split.

Many of the functions from avfilter/formats can return errors, usually AVERROR(ENOMEM).
This propagates the return values.
All of these were found by using av_warn_unused_result, demonstrating its utility.
Tested with FATE. I am least sure of the changes to avfilter/filtergraph,
since I don't know what/how reduce_format is intended to behave and how it should
react to errors.

A number of compilers, for example those from TI and IBM, choke on
these initialisers. The current style is also quite ugly.

It is not used in any filters currently and is inherently evil. If
passing binary data to filters is required in the future, it should be
done with some AVOptions-based system.
While here:
- add missing .version and .category,
- make .class_name consistent across filters,
- align declarations.

avfilter_default_filter_name() is supposed to access an AVFilterContext
struct, if used with a private struct it will cause a crash since it will
access fields which are non defined in the private struct.
The function is modelled after av_default_item_name(), and will print the
name of the instance filter if defined, otherwise the name of the filter.
This allows to show the instance name in the log, which is useful when
debugging complex filter graphs.
It makes more sense for a bit mask to use an unsigned type.
The change should be source and binary compatible on all
supported systems, hence micro version bump.
Fixes a few invalid shifts.

The new syntax is more extensible and more user-friendly.
This breaks the previous syntax, should not be an issue as possibly
no-one is already using anullsrc.
